|
PHARMACIST(S) NEEDED FOR SPECIALIZED CLINICAL PHARMACY POSITIONS
**Sign-on, Relocation, Student Loan Repayment**
Qualified applicants will possess:
- Current California State license in good standing
- TPN/IV experience is a plus.
- Must have clean background and good prior work history
- Candidates must have the ability to effectively communicate, both written and verbally, using the English language.
- Pharm. D. degree and a Pharmacy Practice Residency Certificate (ASHP Accredited program given priority) or equivalent hospital pharmacy experience preferred (minimum 2 years).
- This position requires experience in a specialty field or completion of a PGY2 (Specialty) Residency
Job Requirements:
- Ability to manage multiple tasks at one time
- Inpatient decentralized setting with 30% clinical and 70% order entry/dispensing
- Must be able to exercise good judgment when working independently
- Interpret and process physician orders
- Pharmaceutical care will include but is not limited to:
- Pharmacokinetic evaluation and monitoring
- chart review
- antibiotic monitoring
- drug information
- automatic renal dose adjustments
- IV-PO switches
- Coumadin consults
- therapeutic interchange and educational services
